Community Access and Participation

NDIS Community Participation Support Across Perth and Western Australia

Community Access and Participation

Being part of your community matters. At True Path Care, we provide Community Access and Participation support for NDIS participants across Perth and Western Australia. Helping you connect with others, explore activities you enjoy, and build the confidence to engage more fully in everyday community life.

Whether you are trying something new or returning to activities you love, we are here to make it happen.

What Is Community Access and Participation?

Community Access and Participation is a funded support category under the NDIS, sitting under Support Category 4: Assistance with Social and Community Participation. It is designed to help participants engage in social, recreational, and community activities that build skills, strengthen connections, and reduce isolation.

Funding can be accessed through two streams:

Core Supports - covers everyday assistance to attend and participate in activities, including support worker time and transport.

Capacity Building - covers skill development that helps you participate more independently over time, reducing the need for ongoing paid support.

Understanding which stream applies to your goals helps ensure your NDIS plan is structured in the most effective way

Community courses and workshops offer a practical way to develop new skills and connect with others. We support participants across Western Australia to access community college programmes, cooking classes, and other structured learning opportunities.

How True Path Care Supports Your Participation

Personalised Activity Planning
We take the time to understand your interests, goals, and comfort level — then identify activities across Perth and WA that genuinely suit you. Support is never one-size-fits-all.

Support Worker Accompanied Outings

Our trained support workers accompany participants to activities — providing physical assistance, social prompts, and encouragement throughout. We advocate for reasonable adjustments with community organisations to ensure you are welcomed and included.

Transport Assistance

Getting there is half the challenge. We assist in transport services to and from community activities across Perth — ensuring unreliable or inaccessible transport never becomes a barrier to participation.

Building Confidence and Independence Over Time

Our long-term goal is always greater independence. We use a gradual, scaffolded approach — supporting participants more intensively at first, then stepping back as confidence and skills grow. Progress is tracked and celebrated at every stage.

Why Community Participation Matters

Research by the National Disability Insurance Agency confirms that social and community participation gives participants a sense of belonging and connection, deeper relationships with friends and family, larger social networks, reduced isolation, and greater confidence in everyday community life.

For participants in Perth and across WA, consistent community engagement also opens pathways to employment, further education, and broader opportunities. The right support removes barriers and keeps them that way.
